This is the form to request the formation of a new team. Non-Management can request the formation of a new team, but if they currently belong to a team, this will result in any, and all, proposed members of the team being removed from their current team. As a result, all members who wish to join a team requested under this form should fill out Transfer Form 10Z-T9B.
